Your Visit with Us
As a new or existing patient you can expect a thorough, comprehensive, and personalized eye exam with Dr. Keith E. Taylor. Initial and annual eye exams are 45-minutes long to enable Dr. Taylor to obtain your complete medical history as well as properly assess the condition of your eyes.
To assist Dr. Taylor in gathering your medical information, print and complete the Patient Registration Form before your appointment. The form includes information about your medical history, as well as acknowledgement of our insurance billing and HIPAA privacy practices. Otherwise, please plan to arrive 15 minutes before your scheduled time in order to complete the necessary paperwork.
Insurance and Vision Plans
Dr. Taylor is a participating Medicare and Massachusetts Medicaid provider. The practice accepts Medicare assignment, and as a courtesy, will file with your secondary insurance, if applicable. We are also a participating provider with most major, commercial insurance plans.

If you need assistance with medical expenses, we are happy to establish affordable payment plans. Payment plans are available at no interest when paid by cash or check on a monthly basis. Please call our office manager, Lisa Glass, for more details.
